Our specialized Cardio-Pulmonary Program is designed specifically for those who are managing the effects of cardiac or pulmonary disease, including:

Pathway to Home

Compassionate Care Resulting in Exceptional Outcomes.

At Reliant Senior Care, we are focused on helping our short-term rehabilitation patients return to their highest level of functioning. Our Pathway to Home program is designed specifically to help ease the transition to home after a stay in the hospital or post-acute care center. Whether following knee surgery or recovering from a prolonged illness, those who turn to Reliant Senior Care for specialized rehabilitation services will benefit from the highest standard of clinical excellence.

We are the most fulfilled and rewarded when we can help a patient return to home successfully. The Pathway to Home program supports us in that effort and provides the tools, training and education needed to help our patients achieve exceptional outcomes. Our ultimate goal is to help our patients return to home . . . and decrease the risk of re-admission to the hospital or post-acute care center.

As a short-term rehabilitation patient prepares for discharge to home, the Reliant Senior Care team develops a personalized plan to graduate the patient into our Pathway to Home program. This specialized program, which is incorporated into a patient’s personalized care plan, requires the patient to take a more independent approach to care to begin preparing for a successful discharge to home.

A Self-Directed Approach.

Through the Pathway to Home program, each patient is empowered and encouraged to be more actively involved in their day-to-day activities and care. By being more engaged in their personal care – from activities of daily living, to medication management, to specific diagnosis-related care needs, to starting a home exercise program – our patients are learning how to safely and effectively care for themselves when they return to home.

A Successful Transition to Home.

Our highly trained care team will continually monitor each patient’s progress and assess his/her level of functioning and degree of independence throughout the course of the program. Our care team, together with the participation of the patient, will determine the readiness for discharge and safe transition back to home.
